How to refer
To refer a patient to The Skin Surgery Centre, please provide the following documents:

- Fax Form
- Patient Demographics (Facesheet)
- Pathology Report
- Pictures of Biopsy Site*
We kindly request color pictures of the biopsy site be emailed to photos@theskinsurgerycentre.com. Please note that black and white images are not sufficient.

After we receive the referral, the patient will receive a call from one of our two schedulers usually withing 2-3 business days; the patient does not need to call our office.
If your office uses Nextech or EMA EMRs, a direct message system can be set up for sending referrals.
If the patient has multiple lesions on their pathology report, please clearly specify which lesion(s) you would like us to treat.
